This project outlines for the design and development of a National Cardiovascular Disease (CVD) Clinical Decision Tool and Referral Network System, specifically tailored for use in rural areas of the Philippines. The project addresses the critical need for accessible and quality cardiovascular care, especially in underserved communities, aligning with the Philippine Heart Center's (PHC) research roadmap to impact national health policies. This initiative aims to empower individuals to assess their CVD risk using a mobile application, and to navigate the healthcare referral system effectively.
